We are seeking a highly skilled and motivated Physiotherapist to join our dynamic team at Sirona care & health CIC. As a Community Physiotherapy Practitioner, you will play a vital role in delivering exceptional community-based therapy, enabling service users to improve their quality of life.
Key Responsibilities- Conduct comprehensive assessments and develop personalized treatment plans for service users.
- Work autonomously, prioritizing workload and supervising assistants and support workers as required.
- Support and guide service users in goal setting, using clinical reasoning and evidence-based practice to deliver effective treatment plans.
- Maintain accurate patient records and communicate effectively with the multidisciplinary team.
- Delegate and supervise tasks and physiotherapy treatment plans to Rehabilitation Workers/Physiotherapy Assistants/Technicians.
- Identify risks and take appropriate action to minimize risk to self, service users, and colleagues.
- Participate in training and service development projects as required.
