Job Summary:We are looking for a compassionate and experienced Medical Social Worker to join our team. The ideal candidate will have a strong background in community-based care and a passion for working with patients and families to achieve optimal health outcomes.
Responsibilities:- Assess patients' social and emotional needs and develop plans to address these needs.
- Collaborate with the healthcare team to provide comprehensive care and support to patients and families.
- Identify and connect patients with community resources and services to promote independence and well-being.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in multidisciplinary team meetings to discuss patient care and develop plans to address complex needs.
- Master's degree in Social Work (MSW) from an accredited program.
- Current licensure as a Licensed Medical Social Worker (LMSW) or Licensed Independent Social Worker (LISW) in the state of South Carolina.
- At least 2 years of experience in community-based care, preferably in a home health setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to travel to patient homes, as assigned.
