We are seeking dedicated and compassionate individuals to join our team as Network Navigators in our Acute Mental Health Wards at Blossom Court, St Ann's Hospital. As a Network Navigator, you will play a vital role in ensuring that each patient's network of care is involved early in admission to inform care planning and timely discharge.
Key Responsibilities- Collaborate with patients and their networks to identify and engage relevant individuals and services to support their recovery.
- Develop and maintain effective relationships with community mental health and substance misuse teams, voluntary sector organisations, and other stakeholders.
- Work closely with ward staff to ensure that patient care plans are informed by their network of care.
- Contribute to the development and implementation of strategies to improve patient flow and reduce delays in discharge.
- Diploma in Mental Health Practice or NVQ level 3 or 4 in Mental Health or equivalent experience.
- Educated to GCSE level or equivalent (with a minimum of 5 A - E qualifications - which must include Maths and English).
- Digital competence: Outlook, Word, data entry in EPR systems and Excel plus internet search capability.
