Gray Healthcare is seeking a dedicated and compassionate Care Assistant to support an individual with a diagnosis of Treatment Resistant Schizophrenia and COPD. The ideal candidate will provide one-to-one support, promoting independence and enhancing the individual's quality of life.
Key Responsibilities
- Provide personalized support to the individual, encouraging participation in community activities and promoting social interaction.
- Assist with daily living tasks, such as personal care, meal preparation, and household management.
- Support the individual in accessing community resources, including healthcare services and social activities.
- Encourage and facilitate the individual's interests and hobbies, such as music and DJing.
- Collaborate with the support team to develop and implement personalized care plans.
Requirements
- 12 hours per week, 12-hour shift, 8am-8pm
- Driver preferred due to the individual's mobility vehicle
