Occupational Therapy Assistant Practitioner

We are seeking a highly motivated and enthusiastic Social Care Occupational Therapy Assistant to join our Integrated Community Occupational Therapy Team at Midlands Partnership NHS Foundation Trust.
Job Responsibilities- Support our Social Care OT service by triaging and processing incoming referrals to ensure they meet the criteria and are suitable for our service.
- Conduct focused initial assessments over the phone and provide basic equipment where appropriate.
- Work with Adults with long-term physical disabilities and social care needs across South Staffordshire, providing equipment and minor adaptations to make their home environment accessible and maintain their independence.
- GCSE in Maths and English.
- A willingness to undertake relevant courses within 12 months of appointment.
- Care Certificate completed in all 15 areas within 12 weeks (pro rata) of commencement of contract.
- BTEC in support worker/OT and/or NVQ level 3 in care.
- Evidence of attendance on relevant courses/training sessions, such as Moving and Handling/Food hygiene.
- Computing skills.
- Experience of working in NHS or social care.
- Experience of assessment and care management.
- Good communication skills - verbal and written.
- Good organisational skills - an ability to manage time, prioritise workload and meet deadlines.
- Ability to use information technology.
- Ability to cope with change and work under pressure.
- An ability to assume responsibility and make decisions using own initiative.
- The ability to work as part of a team.
- Knowledge of local health and social care services.
- Knowledge of OT and its role in the organisation.
