The post holder will provide specialist support to patients with cancer and other life-limiting illnesses, working as an integral part of the team within the Trust. They will act as the first point of contact for patients and the multi-professional team accessing the Specialist Palliative Care service.
Main Responsibilities- Deliver specialist advice and ongoing support to patients and their families.
- Maintain established links between the acute sector and primary care teams for smooth patient care transitions.
- Contribute to the 7-day service, working weekends and bank holidays on a rotational basis.
- NMC Registered Nurse (Adult) Registration.
- Evidence of Palliative/End of Life education or similar.
- Degree/Diploma in Nursing.
- Relevant post-registration experience in cancer, palliative/end of life care, or chronic disease management.
- Demonstrable knowledge in the area of Palliative/End of Life care.
