Job Overview
The post holder will provide specialist support to patients with cancer and other life-limiting illnesses, working as an integral part of the team within the Trust. They will act as the first point of contact for patients and the multi-professional team accessing the Specialist Palliative Care service.
Main Duties
The post holder will undertake a range of duties to ensure the effective delivery of care, including delivering specialist advice and ongoing support to their client group. They will work to maintain established links between the acute sector and primary care teams, ensuring a smooth transition of patient care.
