Transformation Manager

We are seeking a highly skilled and experienced Transformation Manager to join our team at Hywel Dda University Health Board. As a key member of our transformation team, you will play a critical role in driving the integration of health and social care services in Pembrokeshire.
Key Responsibilities- Develop and implement a robust integrated plan to drive service transformation and improvement.
- Work closely with stakeholders to build a rich project portfolio and quality assured programme of delivery.
- Establish organisational relationships necessary to make transformation happen, easing tensions and encouraging joint working.
- Develop measures of cost, performance, and user/patient experience, and manage a bundle of transformation projects.
- Coordinate the programme and guide other team members in addition to delivering your own projects.
Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire, and its bordering counties. We provide primary, community, in-hospital, mental health, and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales.
Requirements- Masters level study or equivalent experience.
- Evidence of continuous project management development, including recent postgraduate study.
- Recognised project management qualification or equivalent demonstrable experience.
- Strong IT skills, including the use of spreadsheets to a high standard.
- Clear understanding of the transformation agenda, opportunities for redesigning services, and the tools/techniques for achieving service change.
- Mentoring/coaching qualification.
- Demonstrable knowledge of quality improvement methodologies.
- Understanding of performance measurement metrics and results-based evidence approaches.
