About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our dynamic team at Stockport NHS Foundation Trust. As a key member of our community healthcare team, you will be responsible for providing high-quality patient care and expertise in care delivery.

Key Responsibilities
  • Work autonomously as an ACP within the specialty, managing a caseload of patients and clinical priorities according to agreed protocols and working practices.
  • Deliver individualised patient care, with direct responsibility for assessment, examination, investigation/diagnostic procedures, and diagnosis of patients with undifferentiated and undiagnosed presentations.
  • Appropriately treat patients, resulting in the safe management and appropriate onward referral or discharge of patients at the end of the care episode.
  • Receive and triage referrals via a variety of sources, including direct patient referral.
  • Participate in ward rounds, patient reviews, and multidisciplinary team meetings.
  • Utilise scope of advanced practice to undertake Non-Medical Prescribing role and advise on medicine management issues associated with the patient group, working within Trust policy for Medicines Management.
Person Specification
  • Professional Registration
  • Non-Medical prescribing course (in eligible professions)
  • MSc in clinical practice
  • Teaching or relevant experience qualification
  • Advanced clinical practice skills
  • Evidence of research and audit
  • Evidence of implementing change
  • Evidence of dissemination of knowledge in wider healthcare organisations
  • Post registration experience at a senior level (band 6 or above) in community or a relevant speciality area
  • Evidence of on-going CPD
  • Evidence of teaching in a variety of environments
  • Ability to work independently
  • Experience of using multidisciplinary approach to patient care
  • Strong leadership skills
  • Excellent communication skills and able to communicate with MDT
  • Able to problem solve and make decisions
