Role Overview
As a Rehabilitation Case Coordinator, you will play a vital role in our Treatment Services Team, engaging with patients and external stakeholders to facilitate their rehabilitation process.
This position involves a diverse range of responsibilities, including:
- Collaborating with leading case management professionals to oversee cases from initial referral to successful resolution, ensuring efficiency and accuracy throughout.
- Interfacing with insurers, legal representatives, employers, and service providers to gather referral information, coordinate treatment, and maintain communication with clients.
- Utilizing our specialized system to document and manage case details, ensuring a seamless experience for all parties involved.
- Building and nurturing relationships to effectively manage expectations and address any concerns related to the cases.
- Employing analytical skills to identify the root causes of any issues and implementing timely solutions.
- Approaching healthcare decisions with empathy and consideration for the perspectives of all stakeholders.
