About the Role:
We are seeking a dedicated and proactive Community Care Support Specialist to contribute to the Short Term Support Service within Adult Services at Lancashire County Council. This position is essential in our mission to prevent unnecessary hospital admissions by swiftly assessing and providing necessary care.
The successful candidate will collaborate with various services to ensure optimal outcomes for individuals both in hospital settings and at home. We value enthusiasm and empathy, and we encourage a person-centered and strengths-based approach to care.
