The integration of Clinical Pharmacy within General Practice is undergoing a significant transformation aimed at enhancing patient-centered care. This initiative aligns with national healthcare strategies that emphasize the importance of pharmacy roles in primary care, focusing on proactive patient management to ensure safety and well-being while alleviating the pressures on general practice.

As a Medication Management Technician, you will play a vital role in supporting clinical pharmacists, community pharmacists, and other healthcare professionals within a multidisciplinary team. Unlike clinical pharmacists, your responsibilities will be carried out under supervision, ensuring the effective and efficient use of medications.

Key Responsibilities

The core duties of the Medication Management Technician encompass clinical, technical, and administrative functions, including but not limited to:

  • Assisting Clinical Pharmacists in delivering pharmacy services effectively.
  • Engaging in medication optimization initiatives to promote safe and cost-effective prescribing practices under the guidance of a GP or Clinical Pharmacist.
  • Supporting medication safety monitoring systems, particularly for high-risk medications.
  • Collaborating with healthcare providers and administrative personnel to address medication-related inquiries.
  • Aiding Clinical Pharmacists in conducting Structured Medication Reviews by organizing necessary monitoring tests.
  • Executing medication optimization tasks, including effective administration and reconciliation during patient transitions of care.
  • Assisting Clinical Pharmacists with data collection, safety tasks, and other relevant searches.
  • Promoting the use of Electronic Repeat Dispensing and the Electronic Prescription Service.
  • Supporting the practice in achieving Quality and Outcomes Framework (QOF) targets and enhanced service delivery.
  • Implementing medication changes in response to alerts and guidance from relevant health authorities.
  • Fostering collaborative relationships with community, hospital, and multidisciplinary team pharmacy colleagues.
  • Participating in practice meetings and patient engagement initiatives to enhance the visibility of the Medication Management Technician role.

  • Willingness to engage in approved training pathways relevant to primary care pharmacy.

Experience

Essential Experience

  • Minimum of two years of post-qualification experience.
  • Understanding of clinical principles and evidence-based healthcare.
  • Experience in managing a variety of clinical conditions.
  • Background in primary care, community, or hospital settings.

Desirable Experience

  • Familiarity with systems that support patient management in primary care.
  • Experience with electronic medical record systems.
  • Strong clinical pharmacy knowledge, including relevant terminology.
