Job Summary
We are seeking a highly skilled Clinical Pharmacist to join our team. As a key member of our multidisciplinary team, you will play a vital role in providing expertise in clinical medicines management, conducting face-to-face structured medication reviews, and managing long-term conditions.
Main Responsibilities
- Provide structured medication reviews to ensure patients receive optimal medication therapy
- Conduct acute medicines request reviews to ensure timely and safe medication management
- Manage medication reauthorizations to ensure continuity of care
- Process hospital discharge/clinic letters to ensure seamless transitions of care
- Action MHRA Alerts to ensure patient safety
- Respond to community pharmacy queries to provide expert guidance
- Contribute to the improvement of health outcomes in long-term conditions as part of the multidisciplinary Primary Care Team
- Perform other general tasks as required to support the team
Person Specification
- Independent Prescriber qualification (essential)
- Previous GP Practice experience (preferred)
- Minimum of 2 years Post Graduate experience (essential)
- Commitment to continuing professional development
- Strong clinical leadership, communication, and interpersonal skills
