About the Role

We are seeking a skilled and dedicated individual to join our team as a Healthcare Assistant. This role is critical in ensuring the safe and efficient discharge of patients from hospital, as well as providing packages of care to enable customers to remain independent in their own homes.

The successful candidate will be responsible for building strong relationships with our providers and colleagues to ensure that services and procedures are in place for successful outcomes. This will involve capturing and recording data to inform future demand for services in the borough.
