About the Role

Berkshire Healthcare NHS Foundation Trust is seeking a highly skilled Dietetic Assistant Practitioner to join our team. As a key member of our clinical nutrition team, you will play a vital role in providing high-quality patient care and support.

Key Responsibilities
  1. Collaborate with registered dietitians to develop and implement tailored nutrition support plans for patients with Home Enteral Nutrition (HEN) and nutrition support (NS) needs.
  2. Manage a defined caseload of HEN and NS patients, providing regular reviews and monitoring treatment plans.
  3. Assess, plan, implement, and evaluate treatment plans for delegated patients, ensuring effective and efficient care.
  4. Collect and analyze audit data to inform practice improvements and optimize equipment usage.
Requirements
  1. Experience and/or knowledge in clinical nutrition, with a commitment to continuous professional development.
  2. Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  3. Flexibility, adaptability, and problem-solving skills, with the ability to work independently and as part of a team.
  4. Advanced written, verbal, and digital communication skills, with the ability to maintain accurate records and reports.
  5. Car driver/owner, with a valid license and ability to use a car for work-related purposes.
