About the Role

Berkshire Healthcare NHS Foundation Trust is seeking a skilled Dietetic Assistant Practitioner to join our team. As a Dietetic Assistant Practitioner, you will work closely with registered dietitians to provide tailored nutrition support advice to patients, visiting patients in their own homes or nursing homes to review and monitor treatment plans, ordering equipment, and day-to-day administration support.

Key Responsibilities

  • Support registered dietitians with dietetic management of delegated Home Enteral Nutrition (HEN) patients in their own homes and nursing homes, and nutrition support (NS) patients in community inpatient wards.
  • Manage a defined caseload of HEN and NS patients under the guidance of a registered Dietitian.
  • Implement and review dietetic treatment plans for HEN and NS patients under the care of the dietetic service.
  • Assess, plan, implement, and evaluate treatment plans for delegated patients.
  • Review patients on delegated HEN and NS caseload: Use judgment to review aims, modify, and adapt treatment plans.
  • Collect and assess audit data to help monitor usage of equipment and implement changes to practice where appropriate.
