The Emergency Department is in search of a proactive and motivated individual who is passionate about patient welfare. Key Responsibilities
As a Patient Flow Coordinator, your role will be pivotal in helping the department meet performance objectives and ensuring timely patient care. You will facilitate patient journeys through the department by:
- Monitoring and validating patients who have exceeded the 4-hour care target.
- Collaborating with various teams to streamline patient processes.
- Communicating test results and directing patients as necessary.
- Arranging transportation and addressing inquiries.
- Keeping the patient management system updated in real-time.
Your role is essential in prioritizing patient care and enhancing their overall experience.

Detailed Responsibilities
- Collaborate with interdisciplinary teams to plan patient discharges effectively.
- Ensure all necessary documentation is completed to facilitate the discharge process.
- Communicate with patients, families, and staff regarding patient care and discharge concerns.
- Coordinate with external agencies and families to ensure effective communication for timely discharges.
- Complete relevant patient documentation to support efficient discharge.
- Validate breaches of the 4-hour target in real-time.
- Assess and document patients' social circumstances and discharge arrangements within 24 hours of admission.
- Monitor the status of referrals and assessments necessary for discharge.
- Follow up on outstanding requests that may delay patient discharges.
- Analyze and interpret information to recommend appropriate actions, seeking guidance from senior colleagues when necessary.
- Assist the Emergency Department team in requesting medications, transport, and discharge summaries to ensure timely discharges.
- Ensure all necessary supplies and documentation for admissions and discharges are readily available.
- Ensure all relevant paperwork is completed by the appropriate personnel for efficient discharges.
- Support the follow-up of test results that may delay discharges.
- Liaise with other care facilities to expedite referrals and assessments, including patient repatriation.
Qualifications & Experience
Essential Criteria
- QCF/NVQ/apprenticeship level 2 in a relevant field (health and social care, administration, business management).
- Experience in a health or social care setting.
- Knowledge of discharge pathways.
- Proven problem-solving skills and the ability to prioritize demanding workloads.
- Understanding of confidentiality principles.
Desirable Criteria
- Level 3 QCF Diploma in a relevant field.
- NVQ Level 3.
- Experience in an acute hospital environment.
- Awareness of community nursing and social care services.
- Familiarity with NHS agendas at both national and local levels.
