We are seeking an experienced and motivated Assistant Practitioner to join our Adult Epilepsy Specialist Nurse (AESN) team. As an Assistant Practitioner, you will play a vital role in supporting the patient pathway from referral to discharge, working closely with registered clinicians to ensure seamless care.
Key Responsibilities- Manage and monitor AESN waiting lists, prioritizing referrals according to service and patient need.
- Assist in organizing and delivering an epilepsy training program for residential and supported living care providers, in collaboration with an AESN.
- Engage in mobile working, being aware of the demand and capacity model to reflect workload needs at any given time.
- Provide support in delivering and reviewing seizure care plans for patients within the caseload.
- Process urgent and routine referrals for AESNs, using clinical and geographical knowledge to assess information and determine priority responses.
- Triage new referrals using a screening tool through direct patient contact via phone.
- Ensure comprehensive detail and adherence to NCHC record-keeping policy when documenting patient information.
- Maintain patient confidentiality and adhere to information governance policy.
- Reprioritize workload as required to meet changing demands and priorities of patients referred to the service.
We are looking for an individual with:
- Foundation degree program or equivalent demonstrable knowledge and skills acquired through training and experience.
- Experience of working with people in the statutory/voluntary/private sector.
- Evidence of ongoing, relevant learning.
- Experience of working in a health or social care setting.
- Highly motivated individual with IT and keyboard skills, ability to concentrate when inputting data/patient records, and good time management.
- Excellent communication skills, with the ability to use tact and diplomacy, empathy and sensitivity, and adapt communication skills to meet individual needs.
- Ability to manage stressful situations, be a team player, and aware of own limitations.
- Able to identify with the Trust's commitment to safeguard and promote the welfare of adults, children, and young people.
