Job Summary

We are seeking a highly skilled and compassionate Clinical Coordinator to join our team at St Margaret's Somerset Hospice. As a key member of our Central Referral Centre, you will be the first point of contact for callers seeking support and guidance.

Main Responsibilities
  • Act as the primary contact for patients, carers, and relatives, providing empathetic and professional support in a sensitive and compassionate manner.
  • Respond to telephone calls and messages, ensuring that messages are promptly relayed to the most appropriate person or service.
  • Liaise with internal and external healthcare professionals to process referrals and answer general enquiries regarding access and referrals to our service.
  • Manage and maintain accurate records of patient referrals, deaths, and discharges on our System One database system.
  • Provide administration support to the wider multi-disciplinary team, including typing of correspondence and documents, filing, photocopying, and distribution.
Requirements
  • Excellent communication and interpersonal skills, with the ability to work effectively with a wide range of people, including patients, carers, and healthcare professionals.
  • Good knowledge of Microsoft Office software packages, email, and the internet.
  • Ability to work well under pressure, prioritise tasks, and manage own workload effectively.
  • Self-aware and committed to self-development, with a positive attitude towards equal opportunities and working in a multicultural environment.
