We are seeking an experienced Adult Nurse Case Manager to join our team at Coburg Banks Technical, a leading provider of complex care services. As a key member of our multi-disciplinary team, you will be responsible for delivering high-quality care to adults with complex needs in their own homes.
Key Responsibilities- Provide face-to-face support to clients and their families, building strong relationships and trust.
- Develop and implement personalized care plans, taking into account the unique needs and goals of each client.
- Train and support a core team of carers with clinical skills and activities, ensuring they have the knowledge and confidence to deliver exceptional care.
- Manage your own caseload of community packages, prioritizing tasks and deadlines to ensure seamless delivery of care.
- Registered Nurse status with a minimum of 12 months' work experience in the UK and an active NMC PIN.
- Experience with acute respiratory care, with a strong understanding of complex care needs.
- Excellent written and verbal communication skills, with the ability to build strong relationships with clients, families, and carers.
- Ability to work flexibly and meet deadlines, with a strong attention to detail and organizational skills.
- A competitive starting salary of £42,000, with opportunities for progression and professional development.
