Nurse Case Manager

We are seeking an experienced Adult Nurse Case Manager to join our team at Coburg Banks Technical. As a key member of our multi-disciplinary team, you will be responsible for providing exceptional care to clients with complex needs.
The Role:- Provide face-to-face support to clients and their families, creating personalized care plans to meet their unique needs.
- Train and support a core team of carers with clinical skills and activities, ensuring high-quality care delivery.
- Manage your own caseload of community packages, working closely with NHS continuing healthcare teams to ensure seamless care coordination.
- Registered Nurse status with a minimum of 12 months' work experience in the UK and an active NMC PIN.
- Experience with acute respiratory care, with a strong understanding of complex care needs.
- Excellent written and verbal communication skills, with the ability to build strong relationships with clients, families, and carers.
- Starting salary of £42,000, with opportunities for progression and professional development.
- 25 days annual leave plus Bank Holidays, with an extra day off to celebrate your birthday.
- Option to swap traditional UK Bank Holidays for cultural days, promoting work-life balance and diversity.
- NVQ support and development, ensuring you stay up-to-date with the latest clinical skills and best practices.
