An exciting role has emerged within the Adult Community Nursing Service. We are currently in search of Band 5 Community Nurses to operate within the Hounslow and Richmond regions. Our service runs from 7 am to 7 pm, providing essential care across these boroughs.

Collaboration with integrated rapid response teams, community matrons, and social service partners is crucial to ensure our patients receive seamless care.

Key Responsibilities

Your role will involve delivering high-quality nursing care and assisting the Community Nursing Team Manager in the ongoing assessment, planning, implementation, and evaluation of the holistic needs of patients, their families, and/or caregivers.

You will provide exceptional nursing support to housebound patients with complex health requirements, enabling them to remain in their homes. This will involve close collaboration with specialist nurses, allied health professionals, and social care teams to address patient needs effectively, minimizing unnecessary hospital visits.

This position requires independent work while being part of the Community Nursing Team, covering a designated geographical area. Responsibilities will include conducting initial assessments and reviews, planning, implementing, monitoring, and evaluating individualized care programs. Flexibility to adapt to the corporate approach to patient care is essential.

Qualifications

Essential:

  • Registered Nurse Level 1 with current NMC Registration and a recognized nursing qualification at degree level
  • Qualification or training in continence/bladder and bowel health, with assessor and supervisory training

Experience

Essential:

  • Significant clinical experience in bladder and bowel health at a senior level, with experience in clinical leadership and supporting others in delivering high-quality care

Desirable:

  • Experience working in community or primary care settings, as well as in multidisciplinary teams
  • Experience in managing and supervising clinical staff

Knowledge

Essential:

  • In-depth knowledge of bladder and bowel dysfunction and its management
  • Leadership and managerial skills to effectively run the service

Skills

Essential:

  • Ability to work independently and collaboratively within a multidisciplinary team
  • Competence in managing complex clinical cases
  • Strong communication skills, both written and verbal

Other Requirements

Essential:

  • Proficiency in IT, including Microsoft Office applications and electronic patient record systems
  • Ability to work under pressure and meet deadlines
  • Effective time management skills
  • Willingness to travel within the service area, with access to personal transport
