About the Role

Central Bedfordshire Council is seeking an experienced manager or senior care worker to join our Community Reablement service. As a key member of our team, you will play a vital role in delivering a successful short-term therapy assessment and care program that supports residents to live well in their own homes.

Our service has actively responded to the pandemic by implementing a rapid Discharge to Assess HomeFirst model. As an integral part of our team, you will work closely with health and social care partners across an integrated care system to design and deliver a social care crisis response model that enables people to remain in their homes, avoiding hospital or residential care settings.

Key Responsibilities
  • Manage and lead a team of care professionals to deliver high-quality care and support services
  • Develop and implement effective care plans that meet the needs of our residents
  • Work closely with health and social care partners to design and deliver a social care crisis response model
  • Ensure compliance with CQC regulatory and Safeguarding frameworks
