About the Role

We are seeking a highly motivated and enthusiastic Stroke Reviewer to join our specialist multi-disciplinary stroke rehabilitation team in Lambeth and Southwark. As a Stroke Reviewer, you will play a vital role in assessing the needs of stroke survivors and providing support to help them achieve their full potential.

Key Responsibilities
  • Conduct comprehensive stroke reviews to identify new or unmet needs and develop appropriate care plans.
  • Work collaboratively with stroke survivors, carers, and healthcare professionals to ensure seamless care coordination.
  • Provide education, advice, and guidance on stroke prevention and self-management.
  • Develop and maintain effective relationships with local agencies and community groups to ensure access to resources and services.
  • Participate in quality improvement activities and contribute to the Stroke Sentinel National Audit Programme.
Requirements
  • English Language and Maths GCSE grade C and above.
  • Formal qualification in relevant subjects, such as NVQ Level 3 or NVQ Level 4.
  • Excellent computer skills, including Microsoft Office programmes.
  • Experience of working with adults with disabilities and/or older people in healthcare or related settings.
  • Experience of working as part of a multidisciplinary team and direct patient care.
