We are seeking a highly motivated and experienced Stroke Reviewer to join our team at Guy's and St Thomas' NHS Foundation Trust. As a Stroke Reviewer, you will play a vital role in providing comprehensive stroke review assessments to service users approximately 6 months post-stroke, using a validated and standardized tool.
Key Responsibilities- Contact service users eligible for a Stroke six-month review and offer an appointment either by phone/virtually, in their home, or at clinic.
- Carry out comprehensive stroke review assessments, developing a particular focus on new needs the service user may have developed since their discharge.
- Provide Stroke prevention education, advice, and guidance to improve self-management and encourage meaningful engagement within their local community.
- Liaise with service users' carers to explore any needs that they have which should be addressed, and clinically reason to determine appropriate onward referrals/care pathways.
- Communicate sensitive and complex information regarding rehabilitation to service users, carers, family, and other health and social care professionals and organizations.
- Screen service users' or carers' psychological state using validated mood screens and offer advice for onward support where needed.
- Complete medication reconciliation to ensure service users are taking the recommended prescription as stated on their hospital discharge report.
- Review walking aids and equipment that has been prescribed previously by clinicians to ensure they continue to meet the patients' moving and handling needs.
- Write a report to the service users' GP highlighting any concerns identified and rationale for actions that need to be completed by the GP.
- Make referrals to other services within the community as per the service users' needs, using analytical and clinical reasoning skills.
- Participate in the triage of referrals, using appropriate rationale to prioritize accordingly.
- Effectively manage own varied caseload, using prioritization and time management skills in a variety of community settings.
