Job Summary:
Leaders in Care is seeking a highly skilled and experienced Adult Nurse Manager to join our team. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans that meet their unique needs.
Key Responsibilities:
- Design and deliver tailored care plans that address the physical and physiological needs of clients
- Provide ongoing monitoring, support, and assessment of clients' needs within your caseload
- Deliver nursing interventions, including the management of medication delivery
- Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
Requirements:
- RGN with an active NMC PIN
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years
- Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs
- Full UK Driving Licence desirable but not essential
