About the Role:

We are seeking a highly skilled Care Assessment Officer to join our team in Sheffield. As a Trusted Assessor, you will play a vital role in promoting ongoing enablement and helping individuals receiving care to reach their full potential for independent living.

Key Responsibilities:
  • Conduct comprehensive assessments and continuous reviews of care needs to ensure that care plans are dynamic and continually evolving to meet changing needs and preferences.
  • Support providers in fulfilling their duties under CQC regulations, identifying and implementing the least intrusive services to boost independence.
  • Proactively address life challenges to maintain quality of life, collaborating with Sheffield City Council, local Primary and Community Health Care networks, and Community Services.
Requirements:
  • A good understanding of the current legislative framework in reference to the Care Act 2014, including eligibility, MCA, and safeguarding.
  • Knowledge of CQC regulations and the ability to recommend changes to care plans and understand the responsibilities and tasks associated with procuring services.
  • A good basic level of education and Customer service NVQ Level 3.
  • A valid UK driving licence and your own transport.
  • The current right to work in the UK and willingness to participate as a secondary back-up for our out-of-office service on a rota basis.
