About the Role

We are seeking a highly skilled and dedicated Adult Dementia CPN to join our team at West London NHS Trust. As a key member of our multidisciplinary team, you will provide specialist assessments, reviews, and nursing interventions to service users accessing our Cognitive Impairment & Dementia Service (CIDS).

Main Responsibilities
  • Conduct comprehensive assessments and develop care plans in collaboration with service users, carers, and other healthcare professionals.
  • Provide expert advice and guidance on dementia care and management to patients, carers, and healthcare staff.
  • Work closely with the multidisciplinary team to ensure seamless care coordination and delivery.
  • Administer and monitor medication, including depot injections, as prescribed by the medical team.
  • Participate in local audit and research projects to enhance dementia care and services.

Person Specification
  • Professionally qualified and registered Nurse with current NMC registration in Mental Health/Adult Nursing.
  • Evidence of continuous professional development and a commitment to ongoing learning.
  • Experience of working with patients and carers with complex health and social care needs, particularly in dementia care.
  • Ability to work within a multidisciplinary team and communicate effectively with patients, carers, and healthcare staff.
