Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to join our team at Hampton Medical Centre. The successful candidate will have a proven track record of working in a GP surgery and will be responsible for providing high-quality patient care.
Main Responsibilities- Provide patients with access to a Nurse Practitioner in surgery and over the phone for all presentations in primary care.
- Control admission and discharge to own caseload through comprehensive assessment, prescription of evidence-based care, and development of ongoing plans for health.
- Support patients to adopt health promotion strategies and apply principles of self-care.
- Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
- Keep accurate patient records on computer systems.
- Disseminate learning and information gained to other team members to share good practice and inform others about current and future developments.
- Manage and assess risk within areas of responsibility to protect staff and patients.
- Participate in the delivery of Chronic Disease Management and in achieving QOF Targets.
- Ensure good communication between team members and other staff, and communicate any changes of ways of working to those affected.
- Registered General Nurse (currently registered with the Nursing & Midwifery Council)
- Recognised ANP qualification at minimum of degree level or equivalent
- Evidence of appropriate continuing professional development in all aspects of the ANP role
- Experience in management of long-term conditions
- Proven ability to evaluate the safety and effectiveness of their own clinical practice
