Key Responsibilities
East Sussex Healthcare Trust (ESHT) is in search of a dynamic and seasoned Social Care Practice Education Lead to join our committed team. This crucial role will span both health and social care organizations, concentrating on the execution of a trusted assessor model. The primary objective is to enhance the efficient flow of patients between the Urgent Community Response Team at ESHT and ASC teams, ensuring smooth transitions and optimal patient care.

The urgent community response team operates across multiple sites. This position will involve collaboration across these locations.

Key Duties
- Implementation of Trusted Assessor Model: Lead the integration of a trusted assessor model to enhance patient flow and improve care efficiency.
- Team Collaboration: Collaborate closely with the Urgent Community Response Team and ASC teams to facilitate hospital discharge, admission avoidance pathways, Homefirst, and virtual ward pathways.
- Multi-Disciplinary Coordination: Engage with a diverse multi-disciplinary team (MDT) consisting of nursing, allied health professionals, administration, and social care staff who provide urgent, short-term support during crises.
- Change Management: Propel change initiatives and ensure effective implementation of new processes across health and social care organizations.
- Continuity of Patient Care: Ensure service users receive appropriate care following initial short-term intervention, whether they return to independent living or require longer-term support.
- Educational Development: Create and implement educational and training programs for staff across health and social care settings to ensure understanding and effective utilization of the trusted assessor model.
Candidate Profile:
- Experience: Proven experience in health or social care settings.
- Leadership Skills: Strong leadership abilities with a focus on effective change management.
- Communication Skills: Excellent verbal and written communication skills.
- Project Management: Experience in project management or relevant project work is highly desirable.
- Team Collaboration: Ability to work collaboratively within a multi-disciplinary team environment.
- Educational Expertise: Experience in developing and delivering educational programs to support staff development and enhance service delivery.
Qualifications
Essential:
- Vocational Qualification Assessor or other relevant assessment qualification.
- Educated to Degree level or equivalent experience in practice.
- Leadership qualification/training or equivalent relevant experience.
- Evidence of relevant CPD to Masters Level.
Desirable:
- Qualification in education or training, e.g., PTTLS / L4 education and training certificate.
Experience
Essential:
- Minimum of 3 years' experience working directly with clients and carers in Adult Social Care settings.
- Experience in a Senior Post within Adult Health or Social Care.
- Experience in risk assessment and management in home environments.
- Experience in conducting complex assessments and making sound judgments.
- Experience in inter-professional collaboration.
- Experience in change management within a service in response to research, policy, guidance, or legislation.
- Experience in managing staff, including their education and training development.
- Experience in supervising and appraising staff.
- Delivering quality services along evidence-informed guidelines.
- Experience in designing and delivering training.
- Skilled in reflective practice.
- Evidence of planning and policy development around education and practice.
Desirable:
- Experience in Service Development.
- Experience within acute and community settings.
- Experience in preparing/presenting service developments at a senior level.
