About the Role
We are seeking a highly motivated and experienced Occupational Therapist to join our dynamic team within Acute Frailty. You will play a crucial role in providing comprehensive rehabilitation services to patients with complex physical, psychological, emotional, sensory, and social needs.
Key Responsibilities
- Conduct thorough assessments of patients' functional abilities and develop individualized treatment plans using standardized and non-standardized assessment tools.
- Implement a range of therapeutic interventions to promote independence, enhance functional skills, and improve overall well-being.
- Collaborate effectively with multidisciplinary teams, including physicians, nurses, physiotherapists, and social workers, to ensure holistic patient care.
- Provide expert advice and guidance to patients, families, and carers regarding adaptive equipment, home modifications, and community resources.
- Demonstrate excellent communication and interpersonal skills to effectively convey complex information and build strong relationships with patients and their support networks.
Essential Skills and Experience
- Registered Occupational Therapist with a valid license to practice.
- Proven experience in providing rehabilitation services within an acute care setting, preferably in frailty.
- Strong clinical reasoning skills and the ability to prescribe appropriate equipment and adaptations for daily living activities.
- Excellent time management and organizational abilities, with the capacity to prioritize tasks effectively.
- Ability to work autonomously and as part of a team in a fast-paced environment.
Desirable Skills and Experience
- Experience in clinical audit and outcome measures.
- Presentation and training skills.
