We are seeking a highly skilled Clinical Pharmacist to join our team at The Station Practice. As a Clinical Pharmacist, you will play a vital role in supporting the practice with medication reviews, queries, and prescribing safety.
Main Responsibilities- Lead and oversee high-risk medication monitoring with the practice administration and in-house pharmacy team.
- Provide a point of contact for the practice for all medicines-related queries for healthcare professionals and patients.
- Support the implementation of effective medicine management within the practice.
- Conduct medication reviews, lead on medication monitoring, and provide medication advice.
- Work closely with GPs to provide medicine-related updates and help with problem-solving.
- Lead quality improvements programs that involve medicines.
- Reconcile medicines from outpatient and discharge letters, including liaison with hospital, community, and primary care colleagues.
- Work with the pharmacy team to deliver the Medicines Optimisation agenda, Prescribing Quality Scheme, QOF, and locally commissioned enhanced services.
- Encourage and support adherence to local formulary guidelines and cost-effective prescribing.
- Review, issue, and convert acute medicine requests into repeat medicines, where appropriate.
- Support Community Pharmacists in ensuring NOMAD or batch prescriptions are up to date.
- Implement and monitor a practice's adherence to a repeat prescription policy.
- Support the Practice Pharmacy Technician with the management of repeat prescription processes and other medicines management-related duties.
- Contribute to repeat prescribing safety by prompting patients for reviews, blood tests, and monitoring.
- Work with the multidisciplinary team to improve medicines management and safety within the practice.

- Mandatory registration with the General Pharmaceutical Council.
- Qualifications & Training:
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Holds or working towards an Independent prescribing course.
- Postgraduate Diploma in Clinical Pharmacy.
- Follows professional and organisational policies/procedures relating to performance management.
- Previous experience in hospital, general practice, or CCG.
- Experience and an awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice.
- Demonstrates ability to integrate general practice with community and hospital pharmacy teams.
- Experience of partnership/collaborative working and of building relationships across a variety of organisations.
- Minimum of 2 years post-qualifying experience.
- In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- Excellent interpersonal, influencing, and negotiating skills.
- Excellent written and verbal communication skills.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
- Recognise priorities when problem-solving and identifies deviations from the normal pattern, knowing when to escalate.

- Able to plan, manage, monitor, and review general medicine optimisation issues in core areas for long-term conditions.
- Able to obtain and analyse complex technical information.
- Able to gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
- Able to identify and resolve risk management issues according to policy/protocol.
- Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions.
- Self-motivation.
- Safeguarding and other mandatory training.
- Access to own transport and ability to travel across the locality on a regular basis if required.
- Postgraduate Diploma in Clinical Pharmacy.
- Completion of CPPE 18-month Primary care pharmacy education pathway.
- Experience and an awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice.
- Understand the principles of research governance.
- Adaptable.
- Basic life support training.
