We are seeking a highly skilled and innovative professional to lead and manage a team of multi-agency professionals, focusing on the needs of adults with physical frailty, chronic illness, and primary mental health needs.
About the RoleThe post holder will be responsible for operationally managing and strategically developing the Integrated Team, working closely with the Cluster Partnership and Primary Care services. The team consists of staff from District Nursing, Social Work, Occupational Therapy, Physiotherapy, Community Psychiatric Nursing, Speech and Language Therapy, Dietetic, and Pharmacy, and links closely with the Third Sector.
Key Responsibilities- Develop and implement seamless pathways of care and support for adults with complex needs.
- Lead and manage a team of multi-agency professionals to provide timely professional support for people's needs.
- Work collaboratively with the Cluster Partnership and Primary Care services to develop innovative and creative ways to support individuals and their families.
- Recognize the changing needs within communities and work to meet them through holistic person-centred practice.
- Enable staff to support the person they are working with and those important to them through holistic person-centred practice.
The post holder will need to have a strong understanding of the needs of adults with physical frailty, chronic illness, and primary mental health needs. They will also need to have experience of leading and managing a team of multi-agency professionals and working collaboratively with partners to develop and implement seamless pathways of care and support.
A criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
