We are seeking an experienced Occupational Therapist to join our Mental Health and Wellness Team in Redbridge. As a key member of our team, you will be responsible for providing specialist functional assessments and implementing community care plans to support individuals with complex mental health needs.

Key Responsibilities
  • Conduct comprehensive assessments of daily living skills using the Model of Human Occupational assessment tools
  • Develop and implement individualized care plans to promote recovery, social inclusion, and relapse prevention
  • Collaborate with multidisciplinary teams to provide holistic care and support
  • Facilitate and provide group interventions to promote social inclusion and recovery
  • Work closely with families and carers to ensure their involvement and support in care planning
