About the Role

As a Community Support Specialist, you will play a vital role in connecting with individuals, families, and carers to understand their needs and provide support. Key Responsibilities

  • Conduct assessments to understand individual needs and develop personalized support plans
  • Work collaboratively with our commissioning team to ensure seamless support delivery
  • Build effective relationships with individuals, families, and carers to understand their needs and preferences
  • Identify and report potential safeguarding concerns, mental capacity issues, and other high-risk situations
  • Manage a caseload and work towards clear objectives

About You

To be successful in this role, you will need to have:

  • A Level 2 qualification in Maths and English
  • Relevant experience working with adults in a social care setting, including knowledge of supported living and tenancies
  • A good understanding of social care organizations and their roles and responsibilities
  • Excellent communication and interpersonal skills, with the ability to build effective relationships
  • Ability to recognize the limits of your accountability and responsibility, and seek support and advice when needed
