About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team at Southern Health NHS Foundation Trust. As a key member of our community mental health team, you will play a vital role in providing high-quality care and support to individuals with severe and enduring mental illness.

Key Responsibilities
  • Co-ordinate the care of a complex caseload of service users, ensuring that their needs are met and their care plans are implemented effectively.
  • Undertake initial and detailed assessments to identify the needs of service users and develop care plans that are tailored to their individual requirements.
  • Provide recovery-focused therapeutic interventions, including physical health monitoring, to support service users in achieving their goals and improving their overall well-being.
  • Work collaboratively with service users, their families, and other healthcare professionals to ensure that care plans are implemented effectively and that service users receive the support they need.
  • Monitor and review the effects of prescribed medication, escalating concerns or seeking advice as necessary to ensure that service users receive the best possible care.
  • Develop and maintain professional relationships with service users, based on principles of recovery and empowerment.
  • Accurately record all contacts with service users in the RiO electronic record, ensuring that all communication is consistent with legislation, policies, and procedures.
Person Specification
  • Registered mental health nurse, social worker, or occupational therapist with a strong background in community mental health.
  • Experience of working with individuals with severe and enduring mental illness, including those with dementia and other complex needs.
  • Ability to work collaboratively with service users, their families, and other healthcare professionals to ensure that care plans are implemented effectively.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality care and support.
  • Full driving license and access to a car for work purposes, as completing home visits is a crucial aspect of this role.
