About the Role

We are seeking a dedicated and compassionate Leaving Care Support Worker to join our team in South Wales. As a key member of our support team, you will play a vital role in empowering young people aged 16+ to achieve independence and thrive in their communities.

Key Responsibilities
  • Provide practical and emotional support to young people transitioning from care to independent living.
  • Travel across South Wales to meet with services, providing support and helping young people develop the skills they need to succeed.
  • Work closely with social workers, educators, and other professionals to create and implement personalized support plans.
Requirements
  • Experience or qualifications within social care or education are highly beneficial.
  • A valid driving license and willingness to travel across South Wales (mileage is paid).
  • Strong communication and interpersonal skills.
  • Ability to build trusting relationships with young people.
  • Knowledge of the challenges faced by care leavers and strategies to support them.
  • Flexibility, resilience, and a positive attitude.
