Position Overview
We are seeking a dedicated and proactive registered pharmacist to join our dynamic team, providing cutting-edge pharmacy services to the community.
In this role, you will engage with patients daily, collaborate within a multidisciplinary team, tackle challenges, and have the opportunity to influence the pharmacy services you provide.
Ideal candidates will be efficient, enthusiastic, and confident in working independently while being part of a supportive team. Strong communication skills are essential, as you will interact with social care, clinical, and community teams, as well as patients and their caregivers.
We are looking for individuals who are committed to enhancing patient outcomes, eager to engage in research, and possess a desire to learn and grow in their profession.
A Master's degree in Pharmacy or equivalent, along with an independent non-medical prescribing qualification (preferably), and registration with the General Pharmaceutical Council is required.
Flexibility in your work approach and the ability to travel are essential, as this role involves hybrid working arrangements.
Experience in community services is beneficial but not mandatory.
Part-time hours may be considered for suitable candidates.
If you are not yet an independent pharmacist prescriber, we will support you in obtaining this qualification. You will start in a band 7 development position, with the potential to progress to band 8a upon successful completion of the qualification and performance evaluation.
Main Responsibilities
Your primary duties will include:
- Collaborating within a multidisciplinary team to support our primary care home teams in delivering patient-centered care.
- Providing clinical input to our rehabilitation and specialist palliative care unit.
- Overseeing a Clinical Pharmacy Service to ensure safe, evidence-based, cost-effective pharmaceutical care.
- Supporting clinical pharmacy services in community hospital wards and specialist palliative care beds.
- Ensuring compliance with medicines legislation and managing risks associated with medication use.
- Leading on responses to drug safety alerts and changes in clinical guidance.
- Providing professional expertise and training on medicines management to healthcare professionals.
- Organizing and participating in regular training and feedback sessions.
- Deputizing for the Head of Pharmacy and Medicines Optimisation when necessary.
