Patient Pathway Coordinator for Children's Continuing Care Team

We are seeking a highly organized and detail-oriented Patient Pathway Administrator to join our Children's Continuing Care Team. The successful candidate will provide administrative support to the team, ensuring the smooth coordination of patient journeys and effective delivery of care.
Main Responsibilities- Provide high-quality administrative and clerical support to the Children's Continuing Care Team
- Coordinate patient journeys, ensuring timely and effective delivery of care
- Facilitate good communication between team members and other departments
- Provide flexible administrative support across the department as required

Person Specification- Minimum of 5 GCSEs
- A Level/BTEC or equivalent experience
- NVQ Level 3 in Business Administration or Customer Care Equivalent
- Experience in dealing with customer complaints and customer care
- Ability to prioritize tasks effectively
- Computer literate, with experience of using a variety of computer programs
- Knowledge of NHS service provision, including the pathway of care
- Knowledge of healthcare administrative systems and processes
- Knowledge of databases, spreadsheets, PAS, Excel, or similar applications
- Knowledge of confidentiality and data protection principles
- Knowledge of Health and Safety
- Knowledge of Equality and Diversity and its application in the workplace
- Experience of using a full range of IT/patient administration systems
- Committed to a 'patient/client first' philosophy
- Confident to work assertively with multi-disciplinary teams
- Team player with a collegial working style and willingness to share responsibility
