Job Title: Support Worker

Acorn Community Care is seeking a dedicated and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in providing training and work placement opportunities for adults with a learning/physical disability.

Key Responsibilities:
  • Hold a relevant diploma in health and social care or be willing to work towards it after completing the care certificate.
  • Take on key worker responsibility for identified customers and attend relevant meetings to support them.
  • Liaise with GP, Dentist, and other health professionals to maintain a good standard of healthcare for the individual customer.
  • Demonstrate sound communication skills, both verbal and written, in accordance with Acorn Community Care's policies and procedures.
  • Support customers to live independently in the community, including assisting with budgeting, devising menus, writing shopping lists, shopping, and cleaning.
  • Accompany customers to access community activities, attend day services, and follow day service allocations.
  • Report any inconsistencies with support and delivery to service managers immediately.
  • Support customers to take prescribed medication appropriately and record any incidents on relevant forms.
Requirements:
  • Competent in health and safety matters and able to effectively manage personal safety and the safety of the person being supported.
  • Ability to work as part of a team and develop and implement appropriate training needs for customers.
  • Competent in using local buses and able to attend day services.
  • Ability to support customers to manage their finances and maintain a good standard of health and wellbeing.
