Job Summary

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Primary Care team in West Birmingham. As a key member of our Neighbourhood Mental Health Team, you will work closely with GPs and other primary care professionals to provide mental health triage, assessment, and outcomes-based intervention to meet the needs of service users.

Key Responsibilities
  • Provide mental health triage, assessment, and outcomes-based intervention to service users
  • Identify need and provide support, advice, or intervention to promote psychosocial wellbeing
  • Ensure robust communication across mental health providers, VCSE organisations, and primary care
  • Participate in local neighbourhood MDTs and provide support and advice to colleagues as required
Requirements
  • Registered Mental Health Nurse/OT/Social Worker
  • Relevant post-registration experience in mental health
  • Understanding of responsibilities under the Care Programme Approach, Clinical Governance, Risk Assessment, and Child Protection Legislation & safeguarding
  • Experience of working in community mental health services
  • Comprehensive assessment skills and risk management skills
