Job Summary:
We are seeking a highly motivated and experienced Care and Support Specialist to join our team at Tameside and Glossop Integrated Care NHS Foundation Trust. As a Care and Support Specialist, you will be responsible for providing high-quality care and support to individuals in their own homes and community settings.
Key Responsibilities:
- Provide personal care and support to individuals, including bathing, dressing, and toileting
- Assist with daily living tasks, such as meal preparation and cleaning
- Support individuals with their medication and healthcare needs
- Work collaboratively with other healthcare professionals to ensure seamless care and support
- Develop and maintain relationships with individuals, their families, and carers
- Contribute to the development and implementation of care plans
- Monitor and evaluate the effectiveness of care and support
- Participate in training and professional development opportunities
Requirements:
- Qualifications in health and social care or a related field
- Experience of working in a care and support role
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility and willingness to work varied shifts
