We are seeking a highly motivated and compassionate Associate Practitioner to join our team at Essex Partnership University NHS Foundation Trust. Key Responsibilities- Work collaboratively with colleagues to deliver effective care and support to patients, ensuring their physical and psychological needs are met.
- Contribute to the development and implementation of individual care plans, working closely with patients, families, and other healthcare professionals.
- Provide therapeutic support and interventions to patients, promoting their independence and well-being.
- Participate in the planning and organization of therapeutic and social activities, ensuring a safe and supportive environment for patients.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Contribute to the maintenance of accurate and up-to-date patient records, adhering to confidentiality and data protection policies.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices.
