Responsibilities
  • Conduct comprehensive Continuing Healthcare assessments to determine eligibility for NHS-funded care.
  • Develop and implement individualized care plans that align with patient needs and goals.
  • Collaborate effectively with multidisciplinary teams, including clinicians, social workers, and other healthcare professionals.
  • Provide expert guidance and support to patients, families, and carers throughout the Continuing Healthcare process.
  • Stay abreast of current legislation, policies, and best practices related to Continuing Healthcare.
Qualifications and Experience

We are looking for a highly motivated individual with:

  • A degree or equivalent level of experience in health or social care.
  • Evidence of professional development and registration with the relevant professional body.
  • Demonstrable experience in Continuing Healthcare assessments and service delivery.
  • A strong understanding of NHS Framework for Continuing Healthcare and Section 117 aftercare.
