Key Responsibilities- Provide person-centred support to individuals in a recovery-focused environment, promoting independence, tenancy sustainment, and positive risk management.
- Enable service users to make progress in their personal recovery journey, attaining goals related to mental health and wellbeing, independence, and quality of life.
- Support individuals to move into their new homes and become as independent as possible in managing their tenancies and daily living.
- Provide psychological and practical support to enhance self-esteem and confidence, empowering individuals to make informed choices and enjoy a fulfilling life.
- Develop supportive relationships with individuals, their families, social workers, NHS professionals, and voluntary agencies, working collaboratively to achieve positive outcomes.
- Contribute to needs/risk assessment and the development of person-centred support plans, demonstrating an understanding of mental health needs and the challenges experienced by individuals who may have experienced trauma and adverse life events.
- Experience in supporting individuals with mental health needs and other challenges.
- Good verbal and written communication skills, with the ability to liaise effectively with external agencies.
- Ability to work in a reliable and accountable manner, with a commitment to equality and inclusive practice.
- Understanding of mental health needs and the challenges experienced by individuals who may have experienced trauma and adverse life events.
