Hours Per Week: 36 hours per week
The Role
Tameside Children's Services is on the lookout for dedicated Transition Support Specialists to join our team as part of the Staying Close initiative. This innovative service is designed for young individuals who have transitioned out of care and previously resided in children's homes, providing them with essential practical, employment, emotional, and social support as they navigate life after care.
The objective of this program is to empower each young person with the necessary skills to lead a successful and fulfilling life once they move into independent living. Support will be extended to these individuals until they reach the age of 21.
About You
We seek Support Specialists who possess vision and enthusiasm to foster positive, authentic relationships with young individuals. You will have high aspirations for the young people you assist, ensuring they have access to a variety of opportunities to cultivate their interests and skills.
Knowledge in developing independence skills such as budgeting, cooking, shopping, DIY, managing bills, and liaising with utility and housing providers is essential to effectively support the young people in this program.
The service operates seven days a week and includes evening shifts.
