About the Role

We are seeking a dedicated and compassionate Link Worker to join our team at the Bathgate Partnership Centre. As a MacMillan Improving the Cancer Journey (ICJ) Link Worker, you will play a vital role in supporting individuals affected by cancer, their families, and carers.

You will work closely with our partners across health and social care, local authority, and the third sector to ensure that no one faces cancer alone. Your primary responsibility will be to provide person-centred emotional and practical support to those in need, using Macmillan's Holistic Needs Assessment Tool to identify and address their concerns.

Key Responsibilities
  • Provide emotional and practical support to individuals affected by cancer, their families, and carers.
  • Use Macmillan's Holistic Needs Assessment Tool to identify and address the needs of service users.
  • Work collaboratively with partners across health and social care, local authority, and the third sector to ensure seamless service delivery.
  • Develop and maintain relationships with local services and organizations to provide appropriate signposting and referrals.
  • Contribute to the development and implementation of ICJ services in West Lothian.

  • Excellent communication and interpersonal skills.
