We are seeking a highly skilled and dedicated Community TB Nurse to join our team at Central and North West London NHS Foundation Trust. As a Community TB Nurse, you will play a vital role in delivering high-quality care to patients with tuberculosis (TB) in their own homes and hospital clinics.
Key Responsibilities- Deliver comprehensive care to patients with TB, including assessment, diagnosis, treatment, and follow-up.
- Work closely with multidisciplinary teams, including healthcare professionals, social workers, and community health workers, to ensure seamless care coordination.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Provide education and support to patients and their families on TB prevention, treatment, and management.
- Collaborate with local health authorities, community organizations, and other stakeholders to promote TB awareness and prevention.
- Conduct TB screening and testing in community settings, including schools, workplaces, and other public areas.
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
- Registered Healthcare Professional with relevant professional body (1st Level Nurse/NMC)
- Degree in a health-related subject or willingness to complete a degree within 3 years of appointment
- Experience of recent community/acute healthcare gained through education or work experience placement or previous employment
- Experience with use of IT systems, including Microsoft, Electronic Patient Records systems, SystmOne
- Ability to organize own work and work as a team
- Ability to cope with change and use own initiative
- Understanding of the training needs of the post and gaps in knowledge
