We are seeking a qualified Health Visitor to join our New Forest East Health Visiting Team. As a Health Visitor, you will play a vital role in delivering the Healthy Child Programme, particularly in the 6 High Impact Areas, using evidence-based practice.
Key Responsibilities
- Lead the delivery of the Healthy Child Programme, focusing on the 6 High Impact Areas.
- Develop and implement effective assessment skills to support caseloads.
- Collaborate with partners to develop personalized care plans.
- Contribute to the development of the team's approach to community-based public health programmes.
Requirements
- Registered Nurse/Midwife.
- BSc Hons Public Health Specialist Practitioner (Health Visiting) or equivalent.
- NMC Registered Part 3.
- Nurse Prescriber.
- Full UK Driving Licence and access to a car.
