Position Overview

Barking, Havering and Redbridge University Hospitals NHS Trust is dedicated to transforming the care model for our growing elderly and vulnerable population by adopting a more anticipatory care strategy. Our goal is to identify individuals at risk of developing frailty and implement early interventions to enhance management, prevent decline, and potentially reverse frailty levels. Central to this initiative will be a comprehensive multi-disciplinary geriatric assessment.

We are introducing additional measures to proactively support patients who may be approaching a crisis that could lead to hospital visits. This includes offering urgent assessment appointments on the same or next day. Furthermore, for patients requiring short-term intravenous therapy, we will provide a limited number of infusion chairs to swiftly address this need.

Key Responsibilities

As a vital member of our team, you will deliver a specialized outpatient and urgent assessment service within the Geriatrics and Frailty departments. This role necessitates close collaboration with consultants, patients, nursing staff, therapy teams, support services, outpatient managers, and the Service Manager.
